For individuals with profound and multiple learning difficulties (PMLD), a higher level of care and support is often required, including assistance with mobility, personal care, and communication.

Our services are customized to meet the unique needs of each individual, allowing us to effectively encourage and empower them while providing necessary support. Each person’s journey is guided by a comprehensive Support and Goal plan, which they
actively contribute to defining.

Transitioning to Supported Living

We take special care to support individuals transitioning into adulthood by implementing personalized support and goal plans to achieve sustainable outcomes. Emphasizing inclusion to mitigate the risk of social isolation, we facilitate educational and social activities while encouraging individuals to pursue employment opportunities. Each individual is assigned a dedicated key support worker to foster a meaningful one-on-one relationship. We collaborate with families and external professionals to develop detailed profiles, ensuring informative Support Plans, Positive Behavioural Support plans, Hospital Passports, and all necessary documentation for lifelong support. This holistic approach fosters trust and provides vital continuity of care.

A Multidisciplinary Approach

We advocate for a multidisciplinary approach to support, involving close collaboration with various professionals such as GPs, medical practitioners, speech and language therapists, social workers, and other service providers. This approach ensures that each support program is tailored to the individual's specific needs, considering their preferences, aspirations, goals, and desired level of support hours per week. Support for individuals with autism spectrum disorder (ASD), including Asperger’s Syndrome, is a key aspect of our services at Hallmark Healthcare. Recognizing the uniqueness of everyone with ASD, we conduct an initial 'autism profile' assessment to tailor our support accordingly.

This assessment enables us to understand the specific nuances of the individual’s condition and to provide the most suitable care within the diverse range of autism services available. Our focus is on crafting personalized support plans aimed at helping individuals:
